Management of Illness
To be in compliance with the State of Ohio licensing regulations all children enrolled must have a medical statement on file within 30 days of enrollment. This medical form will be provided at the time of registration and must be signed by a licensed medical practitioner. This form certifies that the child is free from communicable disease and has had the immunizations required by law. For infants who are under one year old, the examination shall occur within 3 months prior to the date of admission. For children who are at least 1 year old, but less than 3 years old, the examination shall occur within 6 months prior to the date of admission. For children who are 3 years old and older, the examination shall occur within 12 months prior to the date of admission.

Children's Enrollment Records
Centers shall maintain enrollment records which include health records, emergency transportation information, and parent or guardian roster permissions for all children attending the center on the form prescribed by the Ohio Department of Job and Family Services.
